Search for Existing Business/Facility Records
NOTE: Manufacturers distributing their own prescription drugs or devices(including virtual manufacturers) must complete a paper application. Please review the instructions for your business model and complete the appropriate application found on the Board’s website at:

  • In order to renew an existing business permit, submit name or address changes or to request permit documents you must create a username and password then associate it to your business records. This form allows you to search for your existing business/facility records.

    NOTE: Your Permit Number is provided to you by the state and is usually printed on your renewal notice. If you have any problems using this system, click here to view contact information for the board. Please omit the '-'(dash) in the fein number during your search.
